SA TOURMED EMERGENCY ASSISTANCE TELEPHONE NUMBERS

This Insurance Certificate certifies that the persons named herein are entitled to the Benefits under this insurance policy subject to the agreed Policy Conditions.

• To enjoy the benefits under this policy, you or the medical service provider must immediately call SA TOURMED at the above emergency numbers for any advice and/or authorization for the required medical treatment
• Have your SA TOURMED Policy Certificate and Passport on hand. You may be required to produce this.
• You will immediately be directed to the nearest appropriate medical facility or be provided with the medical attention or the advice you may require.
• If you have gone directly to a medical service provider eg. doctor or hospital, they must call SA TOURMED at the above numbers after which authorization will be given for the treatment to be administered. SA TOURMED will then settle the claim directly with the service provider.
• If due to the nature of the emergency you are not able to call SA TOURMED or supply the medical service provider with your details, the service provider must inform SA TOURMED as soon as possible of the emergency and obtain the required authorization to ensure payment for the services they have rendered.
• If you do not obtain authorization, you may be required to settle the bill yourself and SA TOURMED may then only reimburse you an amount up to a maximum of R2000. These claims must be forwarded to SA TOURMED within 30 days
• If you lose this certificate, you may reprint one from the SA TOURMED web site by quoting your passport number.
